There are 3 ways to enroll

Use the Vertex GPS Axis Portal

STEP 1

Log on to the portal and begin a new enrollment

STEP 2

Check eligibility and enter enrollment information

STEP 3

Submit for verification and track GPS enrollment status

STEP 2

Enter the Vertex product name in the medication field

STEP 3

Use the search term “GPS” or “enrollment”

STEP 4

Complete the form with your patient and submit electronically

OPTION 3

Download the enrollment form

Fill it out electronically and print, or print the blank form and fill it out by hand

STEP 3

Complete and sign all sections and pages with the patient or caregiver

STEP 4

Fax the completed form to

Please note that Vertex GPS is unable to process incomplete enrollment forms for any of these options.

Once you've completed enrollment, you can notify the patient or caregiver that a GPS Patient Support Specialist will be reaching out to them soon. They're available by phone or text at 1-877-752-59331-877-752-5933 (press 2 when calling), Monday through Friday, from 8:30 AM to 7 PM ET.
